Adoption Home Study Process Administrative needs 

To start the adoption home study process we will need copies (not certified) of the following documents for our files:

• Adoption Application Form
• Birth certificates, marriage certificate, divorce decree(s) if married previously
• Current Medical Form
• Physician's statement about health of children in the home
• Letter from veterinarian stating that pets are current with all immunizations
• Snapshots for our files
• Infertility report (if physician is different from the one who does your routine medical exams)
• Short autobiographies (up to 2 pages each)
• Five reference letters (only 1 from family members allowed) stating how they know you, whether they would recommend you to adopt and why. Please have these letters mailed directly to Cradle of Love Adoption Agency.
• Tax statement (1040) from previous year
• Letter of verification of employment
• If you have a septic tank, Georgia law requires a current inspection

Adoption Home Study Fees:

Domestic Adoption Home Study:

$1800, which includes post-placement visits and a court appearance.

International Adoption Home Study:

$1200; post-placement visits are $200 each; addenda $50 each.

Guardianship Agreement

Cradle of Love Adoption Agency will give you fingerprint cards. Please take them to your local police precinct or the Georgia Bureau of Investigation (GBI). They will fingerprint you.

Then attach a certified check or money order for $15 per person ($30 per couple), and send the cards along with the payment to: Georgia Crime Information Center, P.O. Box 370748, Decatur, Georgia 30037-0748.

Note: They will not accept personal checks. At the time you get your fingerprints done, please ask the police to run a 'Criminal History Check' for you. Many of the international adoption agencies require this document.
